الزمالة في الطب (بالإنجليزية: Fellowship) هي برنامج تدريبي علمي وعملي للأطباء البشريين وأطباء الأسنان يبدأ هذا البرنامج عادةً بعد انتهاء الطبيب من برنامج الاختصاص (الإقامة). خلال فترة التدريب هذه (والتي تكون عادةً أكثر من سنة واحدة) يُطلق على الطبيب مسمى «طبيب زميل» ويكون قادر على القيام بدور الطبيب المعالج أو الطبيب الاستشاري في المجال الاختصاصي الذي تدرب عليه، مثل طب الباطنة أو طب الأطفال. بعد الانتهاء من الزمالة في التخصصات الفرعية ذات الصلة يسمح للطبيب ممارسة العمل دون إشراف مباشر من قبل الأطباء الآخرين, ويحصل الطبيب على شهادة مهنية تؤهله ليكون اخصائياً على مستوى عال.

A fellowship is the period of medical training, in the United States and Canada, that a physician, dentist, or veterinarian may undertake after completing a specialty training program (residency). During this time (usually more than one year), the physician is known as a fellow. Fellows are capable of acting as an attending physician or a consultant physician in the specialist field in which they were trained, such as internal medicine or pediatrics. After completing a fellowship in the relevant sub-specialty, the physician is permitted to practice without direct supervision by other physicians in that sub-specialty, such as cardiology or oncology.

Fellowship es en el ámbito académico de la medicina mundial un periodo de formación para los médicos que ya han realizado un internado o residencia; abarca un tiempo de preparación variable, se considera un programa de especialista porque es post-doctorado. Durante este periodo llevan a cabo el estudio y la práctica de una especialidad médica y al finalizar dicho periodo pasan por un examen obligatorio llamado speciality board examination tras el cual ya pueden disponer del título de médico especialista. No es fácil de conseguir, por lo que se considera un gran mérito la concesión de un fellow.
